AdvantageCare Physicians is participating in The DAISY Award® for Extraordinary Nurses as our way of saying “thank you” to our amazing nurses for the superhuman work they do for patients and families every single day. This award honors nurses wherever they practice, in whatever role they serve, and at any point in their careers – from nursing students to lifelong veterans.

The DAISY Award® highlights the compassionate care and extraordinary moments that frontline nurses are known for. This award celebrates the reasons why most nurses became nurses in the first place, and it reminds them of their impact and importance throughout the year. Please help us honor our nursing staff by submitting your nomination for an amazing AdvantageCare Physicians nurse to receive this award!

The nomination form below asks you to describe how your nominee meets the criteria for The DAISY Award®. Please include detailed examples or stories of how the nominee demonstrated compassionate care, made special connections, or made a significant impact on your life or in a patient’s life.
